Tips on how to submit a
co-curricular Qualtrics form

What you need BEFORE submitting a Qualtrics
1. Preceptors

Preceptors must be registered with Skaggs. If they ARE NOT registered, their names WILL NOT appear on CORE for hours approval. Visit HERE for the general guidelines. Using that link, you can also access the "SSPPS AFFILIATES" sheet. Please use that sheet to check if your preceptors are registered with Skaggs.

If your preceptors are not registered, send them THIS FORM. Here, they will enter their information and resume/CV. Note that the registration process normally takes 3-4 weeks.

If your preceptors are registered but their names do not appear on CORE, please contact Pamela McGlynn.

Make sure you are following the 2:1 students to preceptor ratio when trying to determine the amount of preceptors that you will need.
2. Determine if your event qualifies for SL IPPE/CC.

If your event is an event that has happened in the past, reference THIS to see what it was approved for in the past.
3. The time, date and location.
4. Entry on the Newsletter

Please check with your organization to see who has access to the Newsletter.

To avoid delay in event approval, please ensure that your event is added to the Newsletter BEFORE you submit your Qualtrics.
How to check if your event has been approved

Go to the Newsletter

Find your event on the Newsletter.

Approved events will have comments attached to them. Also, approved domains will be in green. Please see an example below.
In case your event is not approved
You will receive an email asking you to provide more information in order to get the event approved. Please check your email often to ensure your event is approved in time.
This means your event has been approved.
When submitting, note that every submission must be submitted at least 21 days before the actual event. Events are reviewed by the school every Thursday.
Please note that if you still have trouble filling out the Qualtrics, make sure to reference any resources available from your organization.
